The selected team member will provide comprehensive and in-depth nutrition assessments, counseling, and education; will develop and implement a therapeutic nutrition care plan for clients, will participate in the Diabetes Education program, will provide home health nutritional services, and will provide nutrition education within the community. Clients are referred from in-house programs as well as referrals from area health care agencies and providers.
Education and Experience Requirements: Graduation from a four-year college or university with a bachelor’s degree in foods and nutrition, public health nutrition, or dietetics and two years of nutrition experience plus ADA Commission on Dietetic Registration eligible or a master’s degree in nutrition or public health and one year of nutrition experience, plus ADA Commission on Dietetic Registration eligibility; or an equivalent combination of education and experience. Pre-employment drug testing required. Full Salary range: $35,583-$56,932.

Public Health Nurse II

Dare County Department of Public Health is accepting applications for a Public Health Nurse II (PHN II) position. The employee will work as a Family Care Coordinator to clients enrolled in the Community Care Plan of Eastern Carolina (CCPEC) for Dare County and Currituck County. The CCPEC case manager will facilitate client access to those health and social services which enhance the client’s health and well being and his/her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost effective manner possible. Additionally, the PHN II will collect data pertinent to CCPEC initiatives and will participate in network development and public relations for the CCPEC. The position will be based in the Manteo office of the Dare County Department of Public Health but will also conduct home visits as outlined by Dare County Department of Public Health and CCPEC protocols and will travel for required CCPEC meetings and training.
Education and Experience Requirements:
Graduation from a four-year college or university with a B.S. Degree in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or Master’s in Public Health and graduation from a school of professional nursing and one year of professional nursing experience; or graduation from an accredited school of professional nursing and two years of professional nursing experience including one year of Public Health Nursing experience; or an equivalent combination of training and experience preferably with community health nursing and experience. Consideration given for strong computer skills.
Special Requirements:

Responsible for the coordination of Communicable Disease prevention and control programs. Serves as an infectious disease expert with major responsibility for communicable disease reporting, investigation, disease surveillance and tracking, outbreak investigation, and community education and awareness. Will also provide general clinic services coverage for other programs to include STD/HIV, Family Planning, Maternal Health, Child Health, Adult Health, and Breast and Cervical Cancer Control as needed.

Responsibilities include providing supportive personal care to patients in their homes for Home Health and Hospice. Duties also include assisting Registered Nurses, Physical Therapists and other professional personnel in providing care to patients. Considerable knowledge of routine care and treatment of acute and chronically ill clients; working knowledge of general physical problems of acute and chronically ill and obvious signs of condition changes; skill in providing personal care needs of clients, performing nursing assistance acts such as wound care dressings, irrigation of catheters, tube feedings, and enemas as well as assisting with physical exercises. Demonstrated possession of knowledge, skills, and abilities as stated above through at least two years of experience in performing similar tasks in a hospital, clinic, nursing home, medical corps, or medical treatment program. Applicants must be Certified Nursing Assistant I but Certified Nursing Assistant II preferred. Pre-employment drug testing and a valid NC driver’s license are required.
